Legal Disclosure Guidelines for Treatment Providers

I confirm that I am submitting this form as a healthcare and/or treatment provider (“Provider”) and/or authorized representative of a facility (“Facility,” and together with Provider, “Listed Third-Party” and “You” or “I”)
I confirm that I hold all appropriate licenses and am in good standing with all appropriate local, state, and federal licensing and governing bodies.
I confirm that I am not the subject of any investigation, lawsuit, or malpractice claim. I also confirm that within thirty days of my being made aware of any of the following, I will notify NEDA of any changes, including any lawsuits, investigations, malpractice claims, and any change in the standing of, suspension or revocation of any local, state, and federal licensing.
I confirm that I have reviewed and agree to the preceding and following terms.

As a condition to being listed on NEDA’s website (the “Directory”), and in exchange for the benefit of being so-listed, each Listed Thirty-Party Agrees to the following:

It is within NEDA’s sole discretion to choose to list on the Directory any Provider or Facility. NEDA may decline to list, suspend, or remove any Provider or Facility for any reason, at any time, and without any explanation.

NEDA assumes no liability and accepts no responsibility for any acts, omissions, decisions or services attributable to any Listed Third Party on the Directory.

The listing of any Provider or Facility in the Directory is not an endorsement by NEDA of the Provider or Facility nor is it a warranty of the accuracy of the information provided by the Provider or Facility. Providers or Facilities are not permitted to refer to their listing on the Directory as an endorsement by NEDA.

NEDA is not responsible for the accuracy of the information provided in its Directory.

Each Listed Third Party agrees that if any individual accesses the Directory and subsequently asserts any claim against NEDA that in any way involves the information available on the Directory relating to a Listed Third Party (including, for the avoidance of doubt, any claim arising out of any treatment of any kind provided by that Listed Third Party), that Listed Third Party will fully indemnify NEDA for all costs of defense and any damages, and waives any and all claims or counterclaims, whether known or unknown, that it may have had against NEDA. For the avoidance of doubt, Listed Third Parties agree to indemnify and hold harmless NEDA from any and all claims, losses, expenses, obligations, judgments, damages, fines, and liabilities of any nature of whatsoever kind, (including reasonable attorney’s fees) to which NEDA may become subject arising out of or resulting from an individual accessing a Listed Third Party on NEDA’s Directory.

Listed Third Parties release NEDA from any and all liability arising out of, in any way, their listing on the Directory.
